Regarding the fault that has been presented to you what I can tell you is that this hole that you name is a witness hole that carries all the driving water and centrifugal type pumps.
The objective of this hole is to alert that the mechanical water seal of this pump has failed and you run the risk that the pump bearing contaminates you and can cause a much more serious failure in the distribution system.
